• ZOOM STUDIO REGISTRATION FORM & POLICIES
  (Effective January 1, 2026)
• Students should arrive prepared, warmed up, with expected materials at least 2 minutes prior to their assigned lesson/class time. Classes will start and end on time. 
• Students or parent/guardians must complete and sign the Bzdick Studio registration form and agree to all its procedures, permission requests, and waivers.  
• Typically, I do not teach students under 13 years-of-age:
  • I make exceptions to the 13 year minimum on a case-by-case basis
  • Studies show that private vocal instruction is best suited for students above age 13, because this is the age that the voice typically finishes developing. Any type of manipulation to the voice before this time may increase the risk of vocal cord damage. 
  • For students younger than 13, I recommend joining a choir or playing piano with a qualified instructor who will begin working skills like music theory, ear training, and breath control. Advancing these skills early will create an excellent foundation for vocal lessons.​
• Students or parents/guardians must agree to the following payment policies

  MONTHLY TUITION
• Standard Monthly Tuition: $200.00

  PAYMENT SCHEDULE
• Tuition is due on the 20th of each month for the following month.
  (Example: March Tuition is due February 20.)
•

  Students are responsible for knowing their bank’s daily Zelle limits.

  LATE FEES
• Payments received after the due date incur a $15 late fee.
• After 10 additional days, $5 is added for every 10 days late. 
• NON-PAYMENT
  Failure to pay tuition results in:
• immediate pause of lessons
• loss of the reserved time slot
• CANCELLATIONS, MISSED LESSONS, & MAKE-UPS
  NO MAKE-UPS / NO RESCHEDULING
  The purchased service is your weekly reserved time, not a guarantee of four lessons regardless of absence.
• No make-ups are offered for student cancellations.
• No rescheduling is offered for any reason.
• This maintains schedule integrity and fairness for all students. 
  TEACHER ABSENCE OR TECHNICAL FAILURE
  1. Instructor Cancellation
  If I must cancel, a complimentary replacement class/masterclass will be scheduled.
  2. Technical Issues (Remote Learning Clause)
  In the event of a technical issue on my side (internet outage, device failure, Zoom malfunction) that results in:
  MORE THAN 5 MINUTES OF LOST LESSON TIME the session can be continued by phone, or at the instructor’s discretion a credit or reschedule will be offered.
  TERMINATION OF LESSONS
  A 30-day written notice is required to end lessons.
  Notice must be given before the next tuition due date. 
  PUBLICITY RELEASE (Zoom-Specific)
  Instructor will ask for permission before publishing anything.
  I grant Bzdick Studio permission to use:
• Student-submitted pictures and performance videos for promotional materials such as website, social media, or advertising.
  Names may be used unless otherwise requested.
• Check one:

  REMOTE LEARNING LIABILITY WAIVER
  I understand lessons are conducted via Zoom and agree that Bzdick Studio is not liable for:
• internet instability
• audio/video malfunction
• platform outages
• issues with the student’s device or location
• Participation is voluntary and under the responsibility of the student/guardian.
